Furnace MFG Credit Application - Use this form to apply for terms with Furnace MFG.
DVD Master Info Form - If you are sending Furnace MFG a DVD master, please make sure to include this form detailing how your master was authored.
Virginia Resale Certificate - Registered resellers in the state of Virginia should mail in a completed copy of this form to justify exemption from state sales tax.
CD Long Play Waiver - If you are trying to squeeze more than 74mins 40sec of music or 660MB of data into your CD, you must read and sign this waiver explaining possible complications with pushing the capacity envelope.
Furnace MFG strongly recommends you abide by Red/Yellow book standards for capacity in order to avoid playability problems.
DVD Check Disc Waiver - If Furnace MFG is creating a DVD Master for you, you will receive a “check disc” from us. This disc allows you to thoroughly inspect the content, playability and navigation of your new production master before full manufacturing of your project begins. If you wish to forego receiving this important quality assurance step, you must read and sign this waiver regarding possible issues with the finished product.
You will only receive a check disc if Furnace MFG is authoring or modifying your DVD master prior to production. If your master was produced elsewhere, please test it thoroughly before sending it to us. Furnace MFG is not responsible for issues present in third-party masters.
Jewel Case Sticker Form - If you are adding Marketing Stickers to your Jewel Case project, use this form to specify exactly where you would like your marketing stickers applied to your Jewel Case.
